Requirements and Health Forms

Parent Meeting–We invite all new parents to meet with Mrs. Kate O’Brien, Principal, or Mrs. Laura Clark, Assistant Principal as part of the admissions process. These meetings are intended to answer any questions new families may have and to welcome you to HFCA. We would be happy to meet you in person, or we can also arrange a virtual meeting through Zoom. Please contact the school office to schedule. [email protected]

Holy Family Academy embraces diversity. While our Catholic faith is part of our identity, families of all faiths are welcome to join our growing community.

Children must meet the following age criteria for admittance into the following grades:
  • 3 years old on or before September 1st for 3-year-old preschool, and must be potty trained.
  • 4 years old on or before September 1st for 4-year-old preschool.
  • 5 years old on or before September 1st for kindergarten.

These age requirements apply even if the child has previously been enrolled in a school that has made an exception to the deadline. However, we do honor local public school testing results that recommend different placements.

Additionally, students entering HFCA in grades 1 through 8 will be asked to provide current academic information from their former school (student grades, teacher recommendations, and test scores). Our office staff will be happy to help in this process. Please contact Kathy Knuth at 847-907-3481 or [email protected] for more information.
Documentation and Health Requirements:

HFCA requires all new registrants and currently enrolled students to provide a certified birth certificate, baptismal certificate (if applicable), and Illinois Certificate of Health Examination documenting proof of physical and all necessary immunizations and medical, dental, and vision documentation. Please see details by grade.
