Admissions » Requirements

Parent Meeting–We welcome all new parents to meet with Mrs. Kate O’Brien, Principal or Mrs. Laura Clark, Assistant Principal as part of the admissions process.

Holy Family Academy embraces diversity. While our Catholic faith is clearly part of our identity, families of all faiths are welcome to join our growing community.

A certified birth certificate, baptismal certificate (if applicable), and Illinois Certificate of Health Examination documenting proof of physical and receipt of all required immunizations are required for new registrants. Click below for list of Health Requirements (medical, dental and vision documents) required by grade.

Children must meet the following age criteria for admittance into the following grades:
  • 3 years old on or before September 1st for preschool, and must be potty trained.
  • 4 years old on or before September 1st for prekindergarten.
  • 5 years old on or before September 1st for Kindergarten.

These age requirements apply even if the child has previously been enrolled in a school that has made an exception to the deadline. However, we do honor local public school testing results that recommend different placement.

Additionally, students entering HFCA in grades 1 through 8 will be asked to provide current academic information from their former school (student grades, teacher recommendations and test scores). Our office staff will be happy to help in this process. Please contact Kathy Knuth 847-907-3481 or for more information.
